• Photo ID (If Patient Is a Minor, Then a Parent/Legal Guardian ID)

• Co-Pay That Is Due at the Time of Service

• List of Current Medications That You Are Taking and Medical Records From Your Previous Provider, Including Vaccination/Immunization Records

A Few Reminders

• Please arrive 15 minutes early for your appointment.

• Please set your cell phone to vibrate. If you must receive or make a call, excuse yourself to the hallway.

• Due to allergies, only service animals are allowed in our facility. Please leave your companion animals and pets at home.

• Please have the medical assistant check your chart prior to receiving vaccines from a pharmacy or an outside source.

• Please allow 72 hours to process all prescription refill requests once our office has received the request.

• A 24-hour notice is required to cancel or reschedule an appointment. Failure to provide adequate notice will result in a $65-fee.

• Please allow 5-7 business days to process any referral to an outside provider or facility.


The providers and staff at Center Pointe Family Medical Group value your health, time, and patronage. We appreciate your assistance with the above-listed items. Should you have any issues, concerns, or questions, please feel free to reach out to our practice administrator, Tonnie Darling.

Prescription Refill Policy

• Please make an appointment to discuss the need for refills and/or adjustment of medication.

• Call your pharmacy first (including mail order pharmacies) to request a refill. They will contact us electronically, which expedites the refill process.

• Please do not wait until the last minute to call your pharmacy. We require at least 72 hours (3 business days) to fill prescriptions from the time our office receives the request.

• Multiple calls within the 72-hour time frame DO NOT ensure that the request will be processed in a timely manner. In order for the physician to approve the refill request, the patient must have an upcoming appointment scheduled to see their provider.



Note: We encourage you to call your pharmacy prior to picking up your prescription to verify that it is ready. Thank you for working with us to ensure that all your health care needs are met.
